Based on my old discussion with Misiolap.
Imagine a mod to Fallout 2 that would emulate FOnline features and difficulty.

1. You can leave Arroyo only after getting level 21. You need to level on First Aid by letting brahmins/geckos attacking you so you can heal himself. Oops, I forgot - you can level up on Steal too!
2. All containers/shelfs are empty. There is no exception of this rule.
3. Every NPC you encounter got few times more HPs than you and is better armed.
4. Rat or child can knock you out and kill you with one hit even if you wear Combat Armor / Power Armor.
5. Sometimes while entering Klamath/Modoc/Redding you are getting killed by psychotic minigunner.
6. There is random chance that while travelling you will get special encounter: "Caravan". In encounter: 6 guys in Combat Armors/Metal Armors with miniguns/laser rifles and lots of bluesuits in endless turn-based combat as every turn few more bluesuits will enter encounter   .
7. If you are going to fight any NPCs on random encounter expect some dudes in Combat Armors entering it very soon.
8. Randomly before loading a encounter/city map you are laying already dead on ground.
9. There is always chance that some bluesuit will run to you and steal all your all equipment while you are talking / trading with NPCs.
10. If you stand too much in one spot some bluesuit yelling something about Allah will blow you up into pieces... Or that going to be guy with minigun... Or SMG... Or someone with iron fists.
11. If you go into any city in Combat Armor or Power Armor expect to be attacked in few seconds after arrival.
12. If you get a Highwayman and drive into city it will get stolen in a few seconds.
13. While entering to NCR you might randomly die from big explosions taking out of half of city.
14  Sometimes you might stumble across fridge. If you get too close to it / open it you die.
15. Sometimes all NPCs in city you enter are dead.
16. If you tagged Speech then you are newb. If you tagged also Gambling then you are not newb - you are noob!
17. Traders never got anything you need. They mostly got enormous amounts of BB ammo, healing powders and radios.
18. In cities you meet NPCs with bluesuit skins yelling random phrases.
19. When you come back to Arroyo with G.E.C.K you find it razed to ground by NPCs in metal armors with cyrlic names/descriptions.
20. All hostile NPCs are calling you a "n00b" and threat to "p0wn" you.
21. ...

22. All your beloved quest-giving NPC will yell "Fuck your roleplaying you fag, this is the wasteland" when speaked with.
23. The computer chooses randomly names out of a pool, to be displayed over their head. It gets chosen out of a pool with beautiful words like "rape,kill,bitch,kurwa,pwn,n00b" and other mature things.

24. Occasionally a guy with the description of "developer" appears in a town, the dialogue goes as follows, "I'm gonna test something, just stand still for 10 seconds". after 5 seconds he carpet bombs the whole town killing everything and everyone in it, then resurrecting everyone but you.

25. The game will force you to not have a goal, a thing to be worked to, or slightest signs of a story.
26. The game will randomly close itself to emulate the beautiful feeling of the server going down.
27. Sometimes the computer decides to spawn NPC with beautiful cyrillic letters in it which you barely can read.

28. The mod has Lexx instead of President Richardson. Sitting in a big room ful of monitors.
29. Your ultimative goal is to destroy Centaur Bluesuit cloning facility, stop the wipe, hack the server and become a new GM.

30. There is no save/load game functionality. On death, you loose all items on you and are respawn in a nearby location in the wild.

31. While in NCR you might just get to see a parade of strange 3d critters walking through the bazar with no reaction of NPC's whatsoever.
32. Again in NCR you get occasionally robbed and when you try to deliver some swift justice on the thief, you get gunned down by the gurds and looted while the thief walks away.
33. You can build a tent and have someone named "blubber" or "dremor" follow you there, kill and loot you, and the rob your stuff.
34. Occasionaly a NPC in black robes/APA/Frank Horrigan will appear and change your skin to mantis/egg/DC or give away itmes.
35. Every 6 months you loose your character to simulate the server wipe

36. If you park the Highwayman in the wastes, you lose it permanently.
37. If you park the Highwayman in a town, you lose it permanently.
38. All NPCs used aimed shots. On your eyes. No exception.
39. To be able to access the Highwayman you must first speak to Vault City's doctor.
40. Stores only sell BBs, radios and mausers.
41. You are blind beyond 50 metres.

42. You tend to see a guy in a bluesuit with a bunch of holes in it and it would seem he spilled some Cherry Kool-aid on himself and tripped.

44: When you meet an NPC there is a chance that the message 'They're talking to too many people at once. Speak to them later' will pop up, despite the fact that no one else is even in the same room.
45: You can't pickpocket guards or most other NPCs for that matter. You can only steal from children and peasants, both of which yield the bounty of 5 caps. You will often fail at this however, triggering combat and a stream of taunts such as 'n00b', 'Get away from meee...!' and '0MG WTF? WHERED MY CAPSG0??". On the flipside you will often notice a bluesuited man creeping around town, and then notice that all your caps, guns, ammunition and even the armor you had equipped has vanished.
46: When you frequently die you will be teleported to an area with either a bunch of enclave guards, a man and a dog, a caravan or a sergeant surrounded by bluesuited figures, all staring omniously into the distance.
47: When chatting to an NPC that hasn't mysteriously been reduced to a pulp by some Combat Armores hooligan they will give you 30 seconds. If you do not say anything within these 30 seconds then they simply turn away and stare blankly at the wall beside them or at any other items of interest in the room, except for you.
48: Sergeant Dornan from the Enclave Base has been replaced by some person called 'No0bPKerHax0or'. He is now alot more dangerous, less humerous and to be avoided at all costs.
49: Impraper grommerz is Erveywhre! (as wel as typoes...)

50. after you finally found the vertibird blueprints and brought them to frisco (took you 2 hours because of encounters you barely survived) the nice BoS contactman gives you access to the bunker. goin down there nothing is inside the lockers beside a datadisc which reads "We, hereby, are wishing you good luck in finding a PA!" after scanning it with your pipboy.
51. If you ask the computer in that bunker what happended to the nice BoS contactman it plays a video of some guys with miniguns screaming "Nice loot, thanks n00b" while shooting the BoS-Member to tiny shreds.

53. In NCR you may stumble upon people standing around the table trying to sit down.
54. There are no animations, instead, everything is described with pink text above your or npcs' heads.

56. In the middle of a fight you may start moonwalking.
57. Sha Enin gets killed periodically by a guy with the description of "RavenousRat, the bonded and licenced Sha Enin slayer"
58. Your weapon breaks randomly for realism.
