So you were in Golgotha I guess,well then a player or one of your followers could have seen you.
If not then I assume this is a bug,if so then you should report it.

This is not a bug, you get the grave digger karma for digging any grave. It's like child killer, just a fancy title.
Reputation drop is when an NPC will see you - reputation for the faction of the NPC who saw you.
They can see you even through walls though, it's basically a check for all nearby NPCs.
You are less likely to be seen at night, there isn't really any more strategy to it at the moment

You get the Grave Digger perk from the "A Tent in the Wasteland" quest because the rats see you.

If you already killed the rats, it's because ghosts see you.

You get the perk because you have digged someones' grave. It's karma perk not reputation among some folks.

Do you even read what I write? It does not matter if someone saw you or not, you always get it if you dig a grave. Grave Digger is a karma title you get for what you did. There is no consequences for having it, beside some RP factor if you care about it.
