A poison syringe would give PvP a twist, it is easy to put in and in no way unbalanced, it will give antidotes and the snake eater perks much more use.

1 x Empty hypodermic
5 x rad scorpion tails
Damage:
First Aid % divided by 10
You might think that that means it can do 30 damage per second so they will die in 10 seconds, but it is only viable with sneak otherwise you'll be bursted at that range anyway so with 300% sneak the max is 200% FA/10 which is 20 damage per second.
The damage will not go until they are dead and can only be cured by an antidote.
Dead man walking would allow it to only bring you to 1 hp.
Multiple injections will stack so if you are injected 2 times with 300 FA you will lose 60 HP per second.
AP required will be either 4 or 5 and on the item description it will only say ''How useful, this extremely powerful injection does 7 damage!''
And that is referring to it's initial damage on use making it seem useless.
PS. If you inject someone, they will be a dead man walking unless they have dead man walking or antidote, OP right?
Wrong. Once you inject them you are still vulnerable and 1 hex away meaning they can burst you.
