NPC Doctors should dialgue with everyone...

Allow me to introduce Dr. Terry Dix.

A ghoulified doctor in Necropolis, hates smoothskins, talks a lot, insults you, uses questionable methods but will make an exception for such an ugly and stupid smoothskin like you for some caps and a reminder that he swore that oath.

Yeah, if you think about it, most wasteland doctors could only charge a few hundred caps for basic services and wouldn't be too picky about who they take. It isn't like the modern day USA where there's a large elderly population and insurance to support high prices, anything but. The majority of the wastelanders are going to be poor farmers and scavengers, with half of them getting gunned down by raiders before even making it to the sawbones. The Doc might not even have enough patients among the living to be a doctor all the time--they might have a sideline in agriculture, selling drugs, etc.

The whole charisma would better be perk or something.
It works for setting number of allowed followers, but other than that it's just annoyance. I think everyone remember the horrors with Jo - one had to bring character with some charisma, but he couldn't be slaver. Of course mentats worked, but at the same time you couldn't use cigarette which was pretty much must have.

Really cool feature, right?

JohnyBravo, so you think of additional Charisma uses? More dialogue options and goodies for sale. One could barter to heal his ugly follower or arrange a payment with supplies instand of caps, like Chemical Components.

Come to that, doctors in Wasteland will be more inclined to receive raw materials than JMoney because buying them supplies will be quite bothersome for a busy healer.

No, I think charisma would better be "More followers" perk instead, with it's improved versions just like everything else.
If you add more options from it, it'll be just another "make new alt" thing. You must understand what was reason to have charisma in Fallout - choice of your style and option to finish the game in more "talking" way.
Now that's just fine there, it makes good sense, and it was one of the reasons why one could praise Fallout for being so great.
But here you have no objective nor you are really building any reputation, so quests are irrelevant outside of their XP/Money gain. If one wants to be a good guy in Fonline he has to work with people, where any game stat is irrelevant, it's all about player.

The only "solid" reason to ever keep this stat as it is, is purely cosmetic and nostalgic.
