Before you start playing you might want to think bout what role you want to play. For instance I am a crafter that makes armor with a build of 7 STR, 5 PE, 5 EN, 5CH, 10 INT, 6 AG and 5 luck. this is about as crafter as you can go but it really depends on how you want to play. For you it might be a combat build or cross between both. There are many options and paths to choose even being a good doctor could be rewording. My suggestion would be to look at http://www.fo2238.fodev.net/wiki/Main_Page and choose what is best suited to your game play style.

Grommok:
Never carry important items while in town! Sometimes i dont even bring my armor. If you really have to bring some important items (eg caps) keep 'em in your hands.
Build your tent in a location where you can find resources easily (eg near a ruin, so you can find junk, wood, and sometimes fibers and fruits).
Never piss off someone with a plasma rifle.
Rember to carry LOTS of ammo. LOTS of LOTS. Atleast when you plan a few fights.
Outdoorsman is very important, so rember to raise it atleast in the first levels.
In general, TRUST NOBODY. NOT EVEN ME!
