guards should be made human and killable.

patroling guards is also a nice idea

perhaps if your faction raids ncr/hub/whatever enough(or do enough damage) they launch a counter attack on you and your base (nearby tents too of faction members?) if they win they loot what they can and leave :P maybe even kill or steal ur cows :<

say you get a headsup to prepare defences (1hour .. 30 mins?)

edit: sure you'd have to tweak it so alting/whatever to avoid it becomes as much of a hassle as possible, but it would still be good cause then raiding guarded towns is a possibility

I think it would be good if there were just different levels of protection in each city, without any 9999 guards nor some weird gm interventions. It would be only obtained by different strength and quantity of guards.
 So that cities from the south would be hard (Junktown), very hard  (Vault City) and  (relatively) almost impossible (NCR) to raid, and those cities could have call in some reinforcements.

 There could be also some small areas like shops or pubs in north cities guarded by some weaker guards too (like spawn near modoc).
I also think that ordinary NPC citizens should have some basic combat skills and weapons, because it looks stupid when you get chased by a bunch of NPC with their fists, obviously doing no damage.

1st will be : Yay we are safe here [Just like in god damn MMORPG's games like MU/Lineage town are safe]
2nd will be : wait a damn momment aint this is Fallout? we are in post Nuclear World so there are no Rules [ok more civilizated towns]

Why don't they just blow the whole place up, or stay around killing you for the rest of your gaming life? You don't mess with ncr or hub, they have far more menpower and equipment than some random gang of wastelanders.
