u need min 4end and 3ch and being lvl 12. this perk will give u +5hp, and u can take only one time.
u can take it in nr boxing house.

It seems rather pointless to me... +5 hp for everyone (people with low CH/EN after taking some drugs)?

U Have similar perk quest in requiem. +5hp isnt much but for low hp snipers can make a difference.
