What about this suggestion?
When you got HP too low (10-20% from you 100% health)
You got "Fear" status.

Fear effects
You hand shaking and you cannot target fine as before (You weapons hit low 5-15% from you 95% hit)
You run and run, but you cannot escape (Low you AP 3 or 4)

Fear goes off, if Doc helps you (trauma) or you got health more than 10-20%.
