You get items when you start and can do a few quests in the town you start near.
Those quests involve running around the town a bit.
I have to agree it's a bit strange that you start out hiding somewhere in the wasteland, a harsh world hostile people and critters, yet you're unarmed.
Starting with a spear and 1 or 2 bags of healing powder makes a lot more sense.

- go to a random mountain locations, collect 2 flints and xander root and brock flower
- craft weak healing powders (you need 40% first aid, level 1 chars with 6 PE, 7 IN and Good natured trait have 41%)
- craft a primitive tool
- go to a random town ruins location, collect 1 junk and cut a tree with primitive tool
- craft sharpened pole, then craft a spear, then craft a sharpened spear
There you go. You have the items you mentioned. It takes maybe 10 minutes.
One of the biggest mistakes of newbies is false assumption that starting with some better items will make the game easier for them. It won't be easier. Even starting with tent, minigun, combat armor and 10k caps won't make it significantly easier, maybe for a day or two until they lose it all again. If you are unwilling to learn by yourself about the dangers and how to avoid / fight them, if you don't want to die many times in the process of learning, you won't "level up" as a player from newbie. Some additional crappy items at start or "newbie location" won't help here.
What we need is a really good beginner guide / tutorial. There was a guide writing competition. To be honest the guides are mediocre in my opinion, but you can start with reading them. But what I'm afraid is that most new players won't read any guides and continue whining that the game is hard.

This game is cruel to newbies, only if they can't adapt and survive. Hell I can bet psychologically unstable folks would blow their brains out over what's said and done in this game. The best advice I can give you is to watch your own back, be careful who you place your trust in, if the game starts pissing you off, calm down.
If you are unable to calm down on your own take some nerve pills(steal/ask for some from someone with PTSD, or get PTSD by facing a near death experience), and calm down.
New players that survive should use the survival skills of a rabbit, who has to outwit and outrun predators, Remember, when you start out, you are the prey, and you should be wary of predators. Carry minimal gear with you, and avoid other players like you would like to avoid getting warts on your junk, this helps prevent thefts if there's nothing to steal, as well as softening the burn when you die, as you are losing less.
Try to be the bigger man when bad shit happens, because it's going to happen... Alot... Try throwing your murderers off guard, compliment them on the damage done, thank them for the chance to use FA, and if you have high outdoorsman thank them for the EXP you get from dodging encounters getting back to where you want to be. Don't let learning to play the game get to you, trust me, there's a big learning curve I think even the most experienced players haven't gone through it completely.

I'm not saying it will be easier.
Story wise it's strange that you are a traveler hiding from sand storm.
One would expect a traveler in such a world to be armed with a low level weapon like a knife (not the combat one), a sharpened pole, a spear (not the sharpened one) or an old shovel and carrying 1 or 2 healing powders or an antidote.
You would expect travelers in such a harsh world to arm and protect themselves as good as possible before traveling.
Thus grabbing the best weapon they have available and some medication.
