Permadeath would be a very good way to play, just so long as a few things were put into play along with it:
1) XP from killing characters near your level or higher. Full looting is already there, and is good, but a little extra incentive wouldn't hurt.
2) Karma loss from killing characters much below your level. Don't be a blue-suit killer, you don't get anything from it, and it's just mean. Possibly a Noob Killer Karma tag that takes away all guard protection and advertises this fact if you do it too often. It could work sort of like the Child Killer tag from the games... that had some real consequences.
3) Real consequences from low Karma, to go along with point 2.
4) More options for winning fights without killing players. This can already be RP'd a little bit, but honestly who ever does?

permadeath would need completly new game to be made
with perks made for fonline, not for f2
with weapons made for fonline, not for f2
with crafting made for permadeath, not for world when you "deathtravel" from respawn to respawn, live being nothing, stuff being all
with people knowing that its better to loose shit than to be killed, with killers knowing that its more profitable to let someone live robbed, but alive - so that guy could reach something - which could be stolen again
with completly new HP system where starting wouldnt be 1/8 of MAX
with world where the cure for radiation isnt suicide...
etc etc etc
