I would say a 1 out of 20 chance of getting one of those.

So something like a 5% chance to get one of those for each NCR soldier with CAMKII you kill.

Considering the odds of having a repair alt or not, that drops your odds to something like 2,6% a.k.a repair screw up.

But all of that isn't as meaningful as it was now that armors are all about bypasses and not DR.
