Bugged NPC want to kill me if i dont look at character

Hello
Why NPC is bugged in almost city ? Everywhere i got neutral karma and for example:
-i want go to hospital in junktown so i enter the location
-i click to the hospital and my character run to hospital
-i'm dead becouse NPC like guards or others want to kill me and i dont know why. (When running i dont have aimed camera on the character and mostly then npc attacks me
Im not sure but propably my karma decrease... By this BUG im dying everyday few times...
Can you tell me why ?

welll....try to put your gun from your hands to inventory....and it will be ok  ;)

LagMaster:
junktown and NCR don't alow the carry of weapons in active slots, read the warnings
