Well the poll that asks one if he or she participated in the survey is just to give you a general idea of how many people have been involved so far. You can perceive it as a progress bar or something like that ;)

Having a poll like this is a nice idea. Not sure if it is possible, but maybe have it so we can see the results ourselves? Also, yes, it needs more questions about more specific activities and PvP, and T-888's question about alts might be a good thing to have too.
