I dont get it i want to have doctor profession but on wiki on 1 page it says thet i must have first aid too and on other page it says only doctor is requied?

I want to go UNARMORD
                  DOCTOR
                  and meybe
                  RIPAIR OR TROWING OR SIANCE (do i need siance for crafting drugs)

50 Doctor required for 1st level; 100 for 2nd level. The other page is probably outdated.

No, you do not need science to craft drugs.

so what shoud i go for 3 skill ?

Tag doctor only if you want to make pure medic or medic that doesn't need high combat skill (like heavy handed unarmed).
With Blessed are the weak perk you won't need doctor much anyway.

Otherwise I suggest First Aid, Outdoorsman, combat skill.
And make sure you will have enough IN to get doctor to 100%.

Perks that are pretty much mandatory for doctor is Medic and Blessed are the weak.
Others depends on what kind of doctor you create.
