In case of game mechanics, yes realism < gameplay. But you are mistaking mechanics with actions of your character.

Nobody forces you to kill president or slaughter brahmins of Westin.
Exactly because of this freedom your characters should take logical consequences of their actions in game world.

Last time I killed Tandi, my rep was messed up pretty bad, if I remember correctly.  So you can't really just kill her, with no consequences.
Maybe killing those cows could be the same way.  You lose 25 rep with each hit, like you do when you hit/shoot an npc in a city.

Hects Hakal:
You can throw brahamin shit at any NPC whitout reaction ;)
