Does childkiller matter?

Does it affect npc reactions or anything else? Giant red blurs over everything make it impossible to aim properly. Also the child hitbox is probably too large, I wasnt even aiming close.

I think it lowers or raises your karma in some places... I am not sure, I never got Childkiller before... except on Fallout 2...  :-\
