You'll get a "clean" dogtag which you can use science to write whatever you want in it, once written you cannot change it.

When looking at the item in the inventory, it would say something like this:

"You see a dogtag

"<player text>"

It weights xxx"



Being able to choose the design of your dogtag would also be a plus, No new designs are needed, if you used a BoS dogtag to craft your empty one then you'll get a clean BoS dogtag to use, if you used the NCR dogtag to craft then you got yourself a clean NCR dogtag and so on...
