Fa & Doctor cooling time?

I was wondering  what is max cooling time on fa and doctor. What skill is it at when they are max cooling time. Is formula for it?

It's more or less accurate, with FA~35 you get a cooldown of 9 minutes, that's not listed there.

Cheezy:
so saying more less cool time, even all way 300.  I want know how accurate this is and also is there one for doctor.

300% fa gives you 55 seconds cooldown as i remember, also doctor and fa cooldowns are exactly the same now.
