Perk: Living anatomy + burst

Hello everybody!
The perk living anatomy is supposed to give +5dmg per attack on a living creature.
Is this per bullet (like bonus ranged damage) or literaly per "attack"?

Of course I checked the wiki. Unfortunately wiki is not always correct, up to date, or very precise. At least not precise enough to take everything literaly.
